<h3>Why You Won't See Me Show Up in Scrubs</h3><p>I made a deliberate decision early on that nobody at Hyndford Hydration would show up in scrubs. It wasn't a complicated decision. It came down to one thing: this is not supposed to feel clinical.</p><p>When someone books a mobile IV or injection service in Nanaimo, they are choosing to have care come to them in their own space. Their home, their hotel room, their event. That space is theirs. The whole point is that they get to be comfortable, relaxed, and in control of their environment. It feels medical. It feels clinical. For a lot of people, that triggers something they were specifically trying to avoid by choosing a service like this in the first place.</p><p>There's also the privacy piece, and I think about this more than people might expect. Not everyone wants their neighbours clocking a nurse at the door and drawing their own conclusions. That's especially true for in-home IV therapy, where the whole point is that care comes to you quietly and on your terms. Showing up in regular professional clothes means nobody outside that home knows anything they weren't meant to know.</p><p>And honestly, it just feels warmer. I want clients to open the door and feel at ease, not like they are receiving a clinical visit. We can have a conversation, settle in, and make the whole experience feel the way it's supposed to: comfortable, unhurried, and genuinely restorative.</p><h3>The clinical standard doesn't change.</h3><p>The training, the credentials, the sterile technique, the medical oversight; all of it is exactly what you would expect from a BCCNM-regulated nurse serving Nanaimo. The Reason We Don't Wear Scrubs for Mobile IV Therapy in Nanaimo

<p>Toradol is one of those medications that people in acute pain have either never heard of or swear by. If you have ever had it administered in an emergency department for a migraine or a muscle injury, you probably already know what it can do. For everyone else, here is a straightforward explanation of what it is, how we use it at Hyndford Hydration, and why having clinical access to it during a mobile IV session is a meaningful differentiator.</p><h3>What Toradol Actually Is</h3><p>Toradol is the brand name for ketorolac tromethamine, a non-steroidal anti-inflammatory drug with potent analgesic properties. Unlike most NSAIDs, which are primarily anti-inflammatory, ketorolac was developed specifically for pain management and produces analgesia comparable to moderate opioid doses without the opioid-associated risks of respiratory depression, dependence, or sedation (Mahmoodi et al., 2024).</p><p>It works by inhibiting prostaglandin synthesis, thereby reducing the inflammatory signalling that drives both pain and associated symptoms such as nausea. The intramuscular route achieves rapid absorption with peak plasma concentrations typically within 45 to 60 minutes of administration.</p><p>At Hyndford Hydration, Toradol is administered intramuscularly only. IV administration is not authorized under our medical directive. It is used routinely in emergency departments for this indication. The StatPearls clinical reference, updated in February 2024, lists migraine, musculoskeletal pain, and sickle cell crises as established emergency department indications for ketorolac (Mahmoodi et al., 2024).</p><p>For acute musculoskeletal pain, ketorolac has been shown to be non-inferior to opioids across multiple studies examining traumatic and non-traumatic musculoskeletal pain, renal colic, and acute pain presentations, with a more favourable side effect profile for most clients (CEEM Journal, 2025).</p><p>For dysmenorrhoea, NSAIDs as a class have the strongest evidence base of any pharmacological approach to primary dysmenorrhoea, with a Cochrane review of 80 randomized controlled trials establishing their effectiveness for menstrual pain management (Marjoribanks et al., 2015). Ketorolac IM delivers this effect at a faster onset than oral NSAIDs, which is clinically relevant when pain is already established, and oral absorption is unreliable.</p><h3>How We Use It at Hyndford Hydration</h3><p>Toradol is available as an add-on to any IV infusion or as a standalone IM injection, administered only on clinical assessment.</p><p>It is not something you book in advance as a fixed service. Your nurse assesses your presentation on arrival, reviews your health history and current medications, confirms there are no contraindications, and makes a clinical decision based on what is in front of them. If Toradol is appropriate for your situation, it can be administered during your visit.</p><p>The presentations where it is most commonly relevant include acute migraine, particularly in combination with our Pacific Reset infusion, which addresses the magnesium deficit and fluid component alongside direct pain management. Post-athletic pain and acute musculoskeletal pain following physical exertion or injury. Hangover-related pain and myalgia. Dysmenorrhoea. General acute pain assessed on presentation.</p><p>Combining Toradol with Pacific Reset for migraine is worth noting specifically. Magnesium has one of the strongest evidence bases for migraine frequency and severity among nutrient supplements, which we cover in our magnesium post. A nurse working without a valid medical directive and NP oversight does not have access to prescription medications. Access to Toradol under clinical oversight is a direct consequence of having a properly structured medical directive and a regulated nursing team.</p><p>For clients managing migraine, acute pain, or dysmenorrhoea, the difference between a provider who can offer clinical pain support and one who cannot is not a small one. Particularly when you are already in pain, and the last thing you can manage is travelling anywhere.</p><h3>What It Is Not</h3><p>Under our medical directive, Toradol is used for acute pain management based on clinical assessment. It is not a treatment for chronic pain conditions, and its availability at Hyndford Hydration does not replace ongoing pain management through your GP or specialist. The contraindication list for ketorolac is meaningful and includes active or historical GI ulcers or bleeding, NSAID or aspirin hypersensitivity, anticoagulant use, significant renal impairment, active cardiovascular disease, and pregnancy, among others. Your nurse reviews all of these before any administration.</p><p>Ketorolac is also not appropriate for repeated use over extended periods. Our protocol limits use to a maximum of two consecutive days per acute episode, consistent with established clinical guidelines on safe ketorolac administration.</p><p>If your pain is medically significant, persistent, or undiagnosed, the appropriate first step is assessment by your GP. Toradol at Hyndford Hydration is for acute pain presentations in clients for whom the clinical picture is clear, and contraindications have been ruled out.</p><p>IV and IM therapy is supportive in nature. Here is a straightforward explanation of what it is, how we use it, and why access to it during a mobile IV session is a meaningful clinical differentiator.</p><h3>What Gravol Actually Is</h3><p>Gravol is the Canadian brand name for dimenhydrinate, an antihistamine with well-established antiemetic properties. It has been used clinically for decades for the management of nausea, vomiting, vertigo, and motion sickness. It works by blocking histamine and acetylcholine receptors in the brain's vomiting centre and vestibular system, reducing the signals that trigger nausea and the urge to vomit (Health Canada, n.d.).</p><p>It is available over the counter in oral form across Canada. It is not added routinely to infusions. It is available when nausea is present or develops during a session, and clinical assessment supports its use.</p><p>As a standalone intramuscular injection on clinical assessment, for clients presenting with nausea, where IV access is not being established for another treatment.</p><p>All administration is on clinical assessment only. Your nurse assesses your presentation, reviews relevant history, including allergies, current medications, and any conditions that affect its use, and makes a clinical decision based on what is in front of them. It is not something you simply add to a booking in advance like a supplement.</p><h3>When Clients Commonly Use It</h3><p>Nausea shows up in a few consistent contexts at Hyndford Hydration.</p><p>Post-illness recovery is one of the most common. When you are coming out of a stomach bug or a period of illness with poor intake, nausea can persist even as you start to feel better. Having access to antiemetic support during a rehydration session means you can actually tolerate the infusion comfortably rather than lying still, hoping it passes.</p><p>Hangover recovery is another practical context. The morning after a big night typically involves a combination of dehydration, electrolyte depletion, B-vitamin loss, and an unsettled stomach. Which infusion suits best depends on what you are dealing with. Pacific Reset addresses fluid replacement, magnesium, and B vitamins directly, making it a practical choice for physical recovery. Harbour Renewal offers broader nutrient replenishment, including Vitamin C, Calcium, Magnesium, and B vitamins, for clients who want more comprehensive support and are up for it. Your nurse can help you work out which makes more sense based on how you are presenting on the day. Either way, if nausea is part of the picture, Gravol is available as an add-on on clinical assessment and can be administered alongside whichever infusion you choose, addressing the nausea and the fluid deficit at the same time rather than waiting for one to settle before dealing with the other.</p><p>Migraine-associated nausea is a third common presentation. Nausea is one of the most debilitating components of a migraine episode for many people, often making oral medication unreliable because it either cannot be kept down or absorbs too slowly when you are mid-episode. Pacific Reset is our go-to infusion for migraine support, built around magnesium and B vitamins with optional pain relief support on assessment. Magnesium has one of the strongest evidence bases of any nutrient for migraine frequency and severity, which we cover in detail in our magnesium post. Gravol, administered IM or IV alongside Pacific Reset, addresses the nausea component directly while the infusion gets to work, rather than waiting for oral medication to be absorbed through a digestive system already compromised by the episode.</p><p>Nausea during an infusion occasionally occurs regardless of presenting symptoms, particularly at higher infusion rates or with certain nutrients. It is not a treatment for any underlying medical condition, and its availability at Hyndford Hydration does not replace assessment by your GP for persistent or medically significant nausea. If your nausea has an underlying cause that warrants investigation, that conversation belongs with your primary care provider.</p><p>There are also clinical situations where dimenhydrinate is not appropriate, including known allergy to dimenhydrinate or antihistamines, certain types of glaucoma, urinary retention, and concurrent use of CNS depressants. Your nurse reviews all of these before administration.</p><h3>Why This Matters for a Mobile Service</h3><p>Most wellness IV providers do not have access to prescription medications under clinical oversight. A nurse who can assess and administer Gravol when clinically appropriate is a meaningfully different service from one who cannot. It is the difference between managing a client's comfort throughout a session and asking them to tolerate symptoms without any clinical support.</p><p>It is also one of the clearest illustrations of why regulated nursing oversight and a valid medical directive matter in mobile IV therapy. The medication does not carry itself. The clinical judgment to use it appropriately, the screening to ensure it is safe for that specific client, and the documentation of its administration are all part of what a properly structured mobile nursing service provides.</p><p>IV and IM therapy is supportive in nature. <p>Biotin is probably the most aggressively marketed beauty supplement. Walk into any pharmacy or health food store, and you will find shelves of biotin-forward hair, skin, and nail products making confident claims about growth, thickness, and radiance. It is a cofactor in several carboxylase enzymes that are critical to cellular function. Keratin, the structural protein that makes up hair, skin, and nails, depends on biotin-dependent metabolic pathways for its production (Zempleni et al., 2009).</p><p>Your body does not store large amounts of biotin. It is obtained through diet, with sources including eggs, meat, fish, nuts, seeds, and some vegetables, and produced in small amounts by intestinal bacteria. True biotin deficiency is rare in healthy adults with a varied diet, though it occurs in specific populations, including people with inherited biotinidase deficiency, those on prolonged parenteral nutrition, heavy alcohol users, and pregnant women (Zempleni et al., 2009).</p><h3>What the Evidence Actually Shows</h3><p>Here is where we diverge from most beauty and wellness marketing.</p><p>The clinical evidence for biotin supplementation in people without a deficiency is limited. A widely cited 2018 review in the Journal of the American Academy of Dermatology examined available studies and concluded that there is insufficient high-quality evidence to recommend biotin supplementation for healthy individuals without an underlying deficiency or pathology (Lipner &amp; Scher, 2018). A 2017 PubMed review found that in all 18 reported cases of biotin use for hair and nail changes, patients had an underlying pathology. All showed clinical improvement after supplementation. The authors concluded that evidence for supplementation in healthy individuals without deficiency or pathology is lacking (Patel et al., 2017).</p><p>A 2024 review in the Journal of Clinical and Aesthetic Dermatology reached similar conclusions, noting that biotin has not been shown to benefit normal, healthy hair in individuals without deficiency (JCAD, 2024).</p><p>There is modest evidence for specific applications. Small studies have suggested oral biotin may improve brittle nail syndrome in some patients, which is likely where the beauty supplement trend originated. Intramuscular biotin, combined with dexpanthenol, improved total hair density and reduced hair fall count compared with baseline in a study of 50 patients with diffuse pattern hair loss (JCAD, 2024). For people with biotin deficiency, brittle nail syndrome, or certain types of hair loss with an identifiable underlying cause, supplementation has a reasonable evidence base. For generally healthy people with normal biotin levels, the evidence for supplementation producing meaningful improvements in hair, skin, or nail quality is thin.</p><p>We are telling you this directly because it is true and because we think you deserve accurate information before spending money on any of our services.</p><h3>Why We Still Offer It</h3><p>Biotin is available at Hyndford Hydration as both an intravenous add-on to any infusion and as a standalone intramuscular injection. For clients incorporating biotin as part of a broader hair, skin, or nail support approach, adding it during an existing visit is the most practical and cost-effective option.</p><h3>Several reasons inform that decision.</h3><p>First, biotin deficiency is underdiagnosed. The populations at risk extend beyond the obvious. People managing gut health conditions, absorption concerns, prolonged medication use (including certain antibiotics and anticonvulsants), or dietary restrictions may have suboptimal levels without a formal diagnosis of deficiency. For these clients, direct IV or IM delivery bypasses the absorption variables that make oral supplementation unreliable.</p><p>Second, the integrative medicine approach we follow at Hyndford Hydration takes lived experience seriously as data. The volume of people who report improvements in hair, skin, and nail quality following biotin supplementation is consistent across populations and cultures. The absence of large-scale controlled trials does not mean nothing is happening. It means the research has not yet been done at the scale required to establish formal clinical recommendations. As we discussed in our post on IV therapy and the skeptics, the absence of evidence is not evidence of absence.</p><p>Third, biotin is a well-established cofactor in keratin synthesis. The biochemical mechanism by which adequate biotin supports structural protein production is not in dispute. The dispute is whether supplementing beyond dietary adequacy in healthy individuals yields clinically measurable improvements. That is a narrower question than whether biotin matters for hair, skin, and nail health, and it is worth keeping the distinction clear.</p><h3>An Important Clinical Note</h3><p>High-dose biotin supplementation can interfere with certain laboratory tests, including thyroid function tests, troponin assays, and hormone panels, potentially producing falsely elevated or falsely low results. This is a clinically significant interaction that is not widely known and warrants clear flagging. If you are having blood work done, particularly thyroid testing or cardiac markers, biotin supplementation should be paused several days beforehand, and your GP or lab should be informed of any recent supplementation (JAAD, 2024).</p><p>This applies to both IV and IM biotin, as well as to oral supplementation.
